The webinar is also intended for other HCPs interested in the management of patients with COPD.

Statement of Need
In recognition of the need to improve care for patients with COPD, in 2011 clinical practice guidelines for the Diagnosis and Management of Stable COPD were published by several leading associations. Since that time, the 2013 Global Initiative for Chronic Obstructive Lung Disease (GOLD) Report was published and several new medications have been approved. This enduring webinar will provide an update on these guidelines and therapies, as well as current evidence-based information about the CMS Core Measures, which need to be thoroughly understood, as the prevention of a hospital admission has a direct effect on minimizing hospital readmission.

Learning Objectives
Upon proper completion of this activity, participants should be better able to:
  • Describe the impact of COPD and the proposed CMS Core Measures and Readmissions Reduction Program related to COPD.
  • Develop an evidence-based approach for performing appropriate assessment to confirm a diagnosis and determine severity of illness.
  • Explain current evidence-based guidelines for developing an individualized maintenance regimen for patients with COPD.
  • Outline strategies to ensure adherence, continuity of care, and self-management for patients with COPD throughout the healthcare system.
